Companies wishing to appy for CSC status will need to download the CSC application provided here. Please print a copy for your records. There are two ways to fill/submit the application:

1. The application can be filled out electronically and submitted via email. There is a 4mb limit on the overall size of the application file. When done, attach your application and all relevant documentation to an email. Please print a copy for your records.

2. The application can be printed and mailed using the postal service. Please include a copy of all documents and your application fees. When done, press the print button at the bottom of the application. Please print a copy for your records. Please email your application to Carla Hurtubise at carla@eta-i.org.
